A mutagen is a physical or chemical agent or substance that causes change in the genetic constitution or make-up of an individual.
Mutagen causes changes in the DNA structure thereby leading to mutations.
A carcinogen is a substance that causes cancer.
Usually, mutations lead to cancer.
Hence, carcinogens are more often than not mutagens.
